“HOMETOWN HEROES” CONTEST

1. HOW TO ENTER THE CONTEST:
a. The United States Air Force (USAF) Thunderbirds “Hometown Heroes” Contest (“Contest”) will begin on Monday April 27, 2009 at approximately 12:00pm (Noon) Central Time (“CT”) and end on Monday May 11, 2009 at approximately 12:00pm (Noon) CT (“Contest Period”).

b. To enter, log onto www.wbbm780.com, during the Contest Period and select the link to the Contest web page hosted by WBBM(AM). Following the online instructions, complete and submit the online entry form during the Contest Period. Each online entry must include the entrant’s first and last name, complete address, date of birth, a valid email address, and daytime phone number, as well as the first and last name of the “Hometown Hero” that the entrant is nominating along with the Hometown Hero Nominee’s date of birth, if known. Each entrant must also complete a brief statement explaining their “Hometown Hero” selection and why the nominee should be recognized and selected to fly with USAF Thunderbirds. Entrants may nominate any individual in the City of Chicago or Cook County, Illinois as a “Hometown Hero” if that individual represents or demonstrates the efforts and sacrifices put forth by America’s Airmen every day and meets the nominee eligibility criteria set by the USAF and listed below. Examples of Hometown Heroes include, but are not limited to: (i) an average citizen who heroically saved a life; (ii) a renowned teacher; (iii) a community humanitarian; (iv) a local firefighter, paramedic or police officer; (v) one who mentors children or volunteers as a coach or tutor in a youth program; or (vi) someone who dedicates their life to working with at-risk children. Nomination statements must be one hundred (100) words or less; entries exceeding one hundred (100) words will be automatically disqualified.

c. Only one (1) internet entry per person and one (1) internet entry per email address are permitted during the Contest Period. For purposes of these Official Rules, the online entry deadline date is 12 Noon Central Time (“CT”) on Monday, May 11th, 2009. Odds of winning depend upon the number of entries received.

d. The authorized account holder is the natural person who is assigned to the email address by the internet access provider, online service provider or other organization that is responsible for assigning email addresses or the domain associated with the submitted email address. All entries become the sole property of CBS Radio Inc. of Chicago, d/b/a WBBM (AM) (“CBS Radio”), and will not be returned. For purposes of these Official Rules, “receipt” of any on-line entry will be solely determined by when the CBS Radio’s website server records the entry data. Any automated computer form reply or receipt (such as one confirming delivery of email) does not constitute actual receipt by CBS Radio of an entry form for purposes of these Official Rules. Use of any device to automate entry is prohibited. Proof of submission of an entry shall not be deemed proof of receipt by CBS Radio.

e. CBS Radio, the United States Air Force, and the Mayor’s Office of Special Events (“Contest Parties”) are not responsible for entries not received due to difficulty accessing the internet, service outages or delays, computer difficulties or other technological glitches. Contest Parties are not responsible for lost, stolen, deleted, garbled, misdirected, incomplete or late entries.

f. Only one winner per household.
2. ELIGIBILITY RESTRICTIONS:
a. The Contest is open to all listeners 18 years and older who reside in the Chicago Metropolitan Area as defined by Arbitron including Cook, Will, Lake, DuPage, McHenry, Kendall, Kane and Grundy counties in Illinois; Lake and Porter counties in Indiana and Kenosha county in Wisconsin. The Hometown Hero Nominee must be a resident of the greater Chicagoland area (Cook County, Illinois) and reside between the ages of 21 and 55 years of age as of August 2009. Elected and appointed officials; unit honorary commanders; newscasters or media and any persons with prior felony convictions are ineligible as Hometown Hero Nominees. Employees of CBS Radio, the United States Air Force, Mayor’s Office of Special Events, the 2009 Chicago Air & Water Show, and their respective parent, subsidiary, affiliated entities, their licenses, affiliated advertising agencies, participating sponsors/promotional partners, other radio stations in the Chicago Metropolitan area, and members of their immediate families are ineligible to participate or win. Subject to all applicable federal, state and local laws and regulations. Void where prohibited.

b. In addition, the Hometown Hero Nominee must meet the following criteria to qualify for the civilian flight with the USAF Thunderbirds. Air Force regulations dictate height, weight and anthropomorphic measurement standards and the following requirements will be strictly enforced.
i. The absolute minimum and maximum weight for fliers is between 103 and 245 pounds. Additional waivers will not be requested for fliers between 140 – 211 pounds;
ii. The approved sitting height range is between 34 to 39.7 inches and the total height is between 64 to 77 inches. (head to butt and head to knee measurements);
iii. Maximum height is 6 feet, 5 inches;
iv. Chest size range is 34” – 48”;
v. Maximum waist size is 38”;
vi. Boot size range is sizes 5 – 13 (men’s sizes); and
vii. Age limit is 55 years old.

c. In order to be considered for the USAF Thunderbirds flight, three (3) Hometown hero Nominees will be selected and each be required to complete an information sheet, an orientation flight health questionnaire based on the Air Force Flying Class II medical standards, and provide a letter from their physicians stating their current prescriptions, any significant medical issues, and state of their general health before they can be considered for their flight. The United States Air Force has the final say on whether or not the selected Hometown Hero Nominee may be permitted to fly with the USAF Thunderbirds.

d. All information submitted in entry forms will be subject to verification. Entrants are required to provide truthful information and WBBM will reject and delete any entry that they discover to be false or fraudulent. WBBM will disqualify any entry from individuals who do not meet the eligibility requirements, and WBBM will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.

e. Listeners are eligible to win a CBS Radio-conducted contest only once every sixty (60) days and a prize valued over six hundred dollars ($600) in a CBS Radio-conducted contest only once every six months. Only one winner per household is permitted in the Contest.

3. PRIZES:

a. One (1) Hometown Hero Grand Prize. One (1) winning Hometown Hero will be given the opportunity to fly with the United States Air Force Thunderbirds during a practice flight before the Chicago Air & Water Show scheduled for August 15 and 16, 2009 and four (4) reserved passes to the Reviewing Stand at the Chicago Air & Water Show on Saturday August 15th, 2009. Flights are offered on a mission-availability basis. Even if the winning Hometown Hero Nominee is approved to fly with the Thunderbirds, there is no guarantee the flight will happen, based on availability of aircraft, weather, and other operational, logistical or medical factors. Grand Prize has no cash or other value.

b. Two (2) Runner Up Hometown Hero Prizes. Two (2) Runner Up Hometown Heroes will be selected in the event the Grand Prize winner does not meet the Air Force height, weight, anthropomorphic or medical standards. Each Runner Up Hometown Hero will receive four (4) tickets each to the VIP seating area at the Chicago Air & Water Show on Saturday August 15th, 2009. Runner Up Hometown Hero Prize has no cash or other value.

c. The entrants who submit the winning nominations may be recognized by CBS Radio, but no prizes will be awarded to the entrants who submit the nominations.

d. Prizes are non transferable. There is no substitution, transfer, or cash equivalent for prizes, except that the Contest Parties may, in their sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the items above and do not include other expenses. Civilian Ride with the Thunderbirds will originate out of the Gary Airport in Indiana. Transportation to and from the hangar is the sole responsibility of the Winner. Other restrictions may apply.

4. SELECTION OF WINNERS:

a. All decisions of Contest Parties with respect to the Contest are final and binding.

b. Three (3) Hometown Hero Finalists will be selected by CBS Radio from all valid nominations received on or around Friday May 15, 2009. Nomination statements will be reviewed and judged by a panel of judges comprised by WBBM and the Chicago Air & Water Show. Judging criteria will be based on the Hometown Hero Nominee’s community involvement, volunteering, mentoring and/or humanitarian efforts.

c. Finalists will be notified by WBBM via email and telephone on Friday, May 15th, 2009. Finalists do not need to be listening or present to win.

d. Finalists must execute and return any required Affidavit of Eligibility/Publicity Release and Release of Liability Prize Acceptance Forms within five (5) days of notification attempt or prize will be forfeited and an alternate finalist will be selected using the same criteria outlined above.

e. Finalists will each be required to complete a USAF issued Information Sheet, Orientation Flight Health Questionnaire based on the Air Force Flying Class II medical standard and a hold harmless agreement. They will also be required to visit their family physician and provide a letter on official letterhead stating their current prescriptions, any significant medical issues and the state of their general health. All nomination packages will be required to be submitted by June 3rd, 2009 for consideration by the United States Air Force.

f. Pending approval by the United States Air Force Thunderbirds, the Thunderbird Public Affairs will notify the nominee approved to fly by the end of July/early August, 2009. Final determination of the eligibility to fly with the USAF Thunderbirds will be determined solely by the USAF.

5. CONDITIONS:

a. Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by CBS Radio in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.

b. The Hometown Hero finalists will be required to sign a publicity release releasing the Contest Parties from any and all claims in connection with the Contest and agree to have their names, voices, photographs, likenesses, or biographical material used in any advertising or broadcasting material relating to this Contest, without additional financial or other compensation prior to acceptance of their prize.

c. Prior to awarding any prize or prize certificate, CBS Radio will require verification of Hometown Hero Finalists winners’ identification by a showing of valid government-issued photo identification. The Hometown Hero finalists will also be required to sign a liability release, agreeing to hold CBS Radio, the United States Air Force, the Mayor’s Office of Special Events, the Chicago Air & Water Show, their parent, subsidiary and affiliated entities, and the officers, shareholders, directors, employees, agents and representatives of each of them (collectively, the “Released Parties”) harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.

d. Contest Parties reserve the right to disqualify any person tampering with the entry process, and the operation of CBS Radio’s website(s) or who is otherwise in violation of the rules. Contest Parties further reserve the right to cancel, terminate or modify the Contest if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures or any sort.

e. Contest Parties reserve the right to make changes in the rules of the Contest including the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of CBS Radio, any competition or prize-related event, flight or travel is delayed, rescheduled, postponed or cancelled, CBS Radio reserves the right, but not the obligation, to cancel or modify the Promotion and shall not be required to award a substitute prize.

f. Failure to comply with the Contest rules may result in disqualification at the sole discretion of Contest Parties.

g. Contest Parties are not responsible for typographical or other errors in the printing, the offering or the administration of the sweepstakes, or in the announcement of a prize.

h. Copies of the Official Rules or for a list of winners following completion of the Promotion (specify which), send a self-addressed, stamped envelope specifying “Official Rules” or “Winner List” to United States Air Force Hometown Heroes Contest Winner List or Official Rules Request, CBS Radio Chicago, Two Prudential Plaza, Suite 1100, Chicago, IL 60601. A copy of the Official Rules and a list of winners (when complete) are also available during regular business hours at the main studio of CBS Radio at the same address. All such requests must be received within 30 days following completion of the Promotion.
